Let There Be White: LUMIFY is the Red Eye Drop Your Eye Doctor Always Wanted for You

If you suffer from chronic bloodshot or red eyes, you're used to hearing a lot of unsolicited comments and questions. "You look tired." "Are you sick?" "Do you have pink eye?" You may have even been incorrectly judged at work or by friends and family as being drunk or smoking marijuana. But for most people with chronic redness, it's a symptom of their personal ocular chemistry and daily environment. Hours of computer use, inflammatory skin conditions like rosacea or eczema, dry environments like sitting under a vent or fan, working around dyes or chemicals, frequent flying, or driving for long hours with the vents in your car blasting air right at your eyes -- all of these common conditions can lead to recurrent red eyes! For many jobs these work environments are ubiquitous and unavoidable, so to combat the appearance of bloodshot eyes, sufferers turn to the pharmacy eye drop aisle. Visine and ClearEyes are household names, but how they work to accomplish red eye relief has left plenty to be desired. Luckily, a new option is no available over the counter that is poised to revolutionize red eye relief for the millions of Americans suffering from the stigma and stereotyping associated with having chronically red eyes!

LUMIFY (brimonidine tartrate ophthalmic solution 0.025%) was FDA approved on December 22, 2017 for the treatment of ocular redness. The drop is a novel approach to treating red eye with less risk for significant rebound redness. If you have historically used red eye drops like Visine or ClearEyes, you may have noticed your eyes look more red a few hours after use. That's because traditional red eye relief drops work by constricting arterioles, decreasing both blood flow and oxygen getting to the ocular tissue. Once the drop's constriction effects begin wearing off, the arterioles rebound in size and get to work fast to deliver the oxygen and inflammatory mediators that were being slowed down by the drop. The result is enlarged artery walls pumping more blood to make up for lost time, which makes your eyes look redder than they did prior to inserting the drop. Unlike previous red eye drops, LUMIFY acts by constricting the venules on the ocular surface, not the arterioles. By working specifically on the venules, oxygen flow from the arterioles is maintained to the ocular surface, preventing the risk of rebound redness!

If you have historically used red eye drops, have you noticed that their effectiveness seems to wear off with time? Where you might have needed 1 drop a day when you first started using the drop, now you might be up to 4 or 5 times a day, and still feeling like your eyes look red. This is a real and serious side effect of traditional red eye drops! Another great benefit of LUMIFY over older red eye drops on the market is that it has less risk of tachyphlaxis, or the drop losing effectiveness with time. Studies have repeatedly shown that as your body is exposed over and over again to nonselective adrenergic vasoconstrictors like those found in Visine, Clear Eyes, and generic red eye relief drops, the body actually downregulates the number of receptor cells for the medication so that the medication's active molecules can't bind! Your body wants to maintain a steady state, called homeostasis, and if it feels it's getting too much of the vasoconstricting signal, it won't let your arteriole walls respond anymore by actually removing the receptor molecules the eye drop would bind to. The result? You keep putting in drops, but don't get any additional relief.

In 6 clinical trials of LUMIFY and over 600 patients, there was no occurrence of tachyphlaxis or rebound hyperemia.

Be Aware of the Risks of Exposure to Preservatives
Because red eye drops also have preservatives in the bottle, there is an additional side effect of toxic reactions on the surface of the eye from using too much of both traditional red eye drops and LUMIFY.

The preservative in LUMIFY is benzalkonium chloride, which studies show can be toxic to the human cornea at even low doses. Benzalkonium chloride concentrations as low as 0.0001% exposed on the eye for as little as 5 minutes will cause some degree of cell damage to the human cornea and conjunctiva cell tissue.

Directions
LUMIFY is FDA approved for use to relieve redness in adults and children aged 5 years and older. Instill 1 drop in the affected eye every 6-8 hours, and do not use more than 4 times a day. Because of the Benzalkonium chloride preservative used, I personally recommend to my patients to use no more than 1 x a day to help reduce the risk of corneal toxicity.

LUMIFY works quickly and lasts! 95% of patients saw improvement in red eye appearance in just 1 minute and 79% of patients maintained red eye relief after 8 hours.

Do not use this drop if:

you are pregnant or nursing (consult your doctor first)
over contact lenses
if redness or eye irritation persists for more than 3 days

Take Home As an optometrist, I'm thrilled to have a red eye drop that I can recommend to patients that offers relief without the side effects that traditional red eye drops have caused. However, those of you suffering from chronic red eyes still may be dealing with an underlying culprit that a red eye reliever alone can't solve. Dry eye syndrome is one of the most common causes of chronic redness, and treating the underlying dry eye issue is the best path towards true relief. Optometry in Action: How to Be an Optometric Advocate and Legislative Keyperson

With rapid changes in healthcare technology and telemedicine, there's perhaps never been a more important time for optometrists to get involved in local and federal legislative initiatives. Deciding you want to make a difference in changing the course and direction of the profession is a vital first step, but the truth is that getting involved with legislative action can be daunting. How do you develop a relationship with your local or state representatives? How do you get your opinions and passion for eyecare and patient advocacy heard?

For the last few years I've volunteered with the North Carolina Optometric Society as a District Keyperson, meaning that for my local area I'm the point person to get in touch with my Senator and House Representative about the issues affecting our profession and patient care. Volunteering was the easy part - I was already a member of the state society, attending local continuing education events and the annual State Legislative Day where optometrists from across the state journeyed to Raleigh to meet with legislators downtown. But after committing to being a Keyperson I realized one huge issue: I had no idea what I was supposed to do. We didn't take any course in optometry school on lobbying or legislative action. I was totally at a loss.

Luckily for me, the great team of advocates and lobbyists with the NCOS have been an excellent guide. So if you've made that leap to volunteer (and congrats and thank you -- optometry needs you!!), here's a primer on how to be an advocate:

1. Find Your Legislators

You can contact your local state optometric association for this information, or head to Open States and type in your address to find your representatives. The legislators you should focus on are those either where you live or where you work; you should be one of their constituents, active in their community.

2. Request a Meeting

This next part may sound crazy, but you need to pick up a telephone and call! To make the initial meeting request, a phone call will go a lot further in creating a relationship than an email will. You will likely speak with a staff member when you call and not the legislator themselves, but don't ignore the staff! They are essential to getting you a meeting, so make sure you connect with them positively and are friendly no matter who picks up the phone.

-Have a specific request for the meeting: "I need 30 minutes to discuss..." Giving specifics helps get you on the books.

-Email a follow-up shortly after you make the phone call to thank them and say you are looking forward to the meeting.

3. Prepare for 1 Main Issue

While there might be a laundry list of items you want your legislator to address, their time is valuable. Focus on one specific issue to make sure your point is heard and that the meeting is seen as a positive time spend for your legislator.

-Leave behind materials that address your main point. Your legislator is meeting with people all day long, but if you leave behind a small packet or papers with clean, easy to follow bullet points they may circle back to your issue later that day when they see it laying on their desk.

-Connect personally by telling your story. Talk about yourself as a small business owner or healthcare provider in the community. Talk about your patients and the issues they are dealing with. Stories of specific people in their community that demonstrate your issue can leave a strong and lasting impression of the issue. Remember your legislators are real people that got into political action because they care about serving their community. There isn't much money in state and local politics. If you connect to them person to person about the community they love and are passionate about, then your message will have a much stronger meaning. Your here because you are passionate about the community too, so you both already have a lot in common!

4. Execute Your Ask

Wrap up your meeting with a specific request. "I need you to..." This is a powerful way of closing your meeting and calling your legislator to action.

5. Follow-Up

Bring out the pen and paper; a personal, written thank you note is an important final step to making your meeting count. Address the note to both the legislator and their staff. Reiterate in your note the support and action you need your legislator to take: "Thank you so much for committing your support..."

6. Circle Back with Your State Association

After you meet with your legislator, it is important to let your local optometric state association know the outcome or issues raised during your meeting. Knowing the legislators opinions and thoughts on the profession or any patient care issues can help your state association in crafting a more effective grassroots message. A quick email to the point person for your state association's advocacy is all it takes!

Expanding Scope: New Optometry Surgical Fellowship

In 1901 when Minnesota became the first state to recognize and legislate the scope of practice for optometrists, ocular healthcare was quite different than today. As technology has changed and improved to provide safer, faster, and more efficient healthcare solutions, medical providers have had to learn and adopt this new technology to best serve their patient's needs. That's the basis of continuing education hours to maintain a medical license after all. But by 1921 when all states in the US had passed legislation recognizing and defining optometric care, the technology that now exists to treat patients could not have been dreamed of or planned for to be written into those original defining laws. That is the situation that optometry faces now in 2018 where the legislation written in each state that defines the practice of ocular healthcare is now also limiting how the profession can best serve patients.

State by state, legislation is being brought forward to expand the role and scope of doctors of optometry to include out-patient lid lesion removal and laser-assisted treatments for glaucoma and post-cataract surgery care. Oklahoma, Louisiana, and Kentucky all currently have legislation providing doctors of optometry the right to use technology considered standard of care for conditions they are trained and educated to treat (like glaucoma and eyelid lesion removal). But with dozens of other states preparing to expand scope of practice laws similarly, the question often posed by legislators or opposing factions has been what standard of training exists to educate optometrists and optometry school students to proficiently perform these out-patient surgical procedures?

This question sparked The Rosenberg School of Optometry in San Antonio, Texas to create the first ever surgical fellowship in optometry. “Our goal with this fellowship program is to provide optometrists with the best possible preparation for introducing surgical care into their practices,” said Timothy Wingert, O.D., Rosenberg’s Dean. “All optometry schools now train their students to perform surgical procedures, but for doctors who have been in practice a few years and recent graduates seeking advanced training this program will give them the opportunity to perform hundreds of procedures under expert supervision.”

One complication existed to Dr. Wingert's plans. At this time Texas is not a state where optometry's legislation provides for the practice of surgical procedures. To allow their students this important hands-on experience and training, they partnered with Dr. Dennis Golden of Golden Eye Associates in Logansport, Louisiana where surgical procedures in optometry have been legislated since 2014. His office provides full scope ocular healthcare, including removal of eyelid lesions, cysts, chalazion, and in office laser procedures for glaucoma (SLT) and post-cataract surgery care (YAG laser capsulotomy). Dr. Golden is a passionate advocate for optometry's role as a primary eyecare provider. In his rural community, himself and fellow optometrists are the only local eyecare option for patients in need. “There are no ophthalmologists around here.” explained Golden. “The closest ophthalmologist is over an hour away, and many of my patients have no means to get there.”

Dr. Katherine Dronka was the first ever surgical fellowship program participant. She was a 2016 graduate of the InterAmerican School of Optometry in Puerto Rico and had already completed a residency in ocular disease at the John D. Dingell VA Hospital in Detroit, Michigan. She was drawn to pursuing the additional 12 month fellowship program because she recognized the impact that surgical procedures would have on the future of eyecare. "This is an incredibly important program as our profession moves to more medical care," Dronka explained. "Gone are the days that 'one or two' provide fully for our patient's needs."

The surgical fellowship is a 12 month program beginning July 1st with a focus on in-patient experience as well as supervising and training optometry school students rotating from the Rosenberg School of Optometry for instruction on state of the art surgical techniques. Dr. Dronka has had extensive opportunities to grow her skills through both primary patient care at Golden Eye Associates, as well as in nursing home care, emergency calls, and through providing care with local hospital privileges. "This fellowship has exceeded my expectations. To date I have performed hundreds of procedures, including iridotomies, capsulotomies, lump and bump removals and periocular surgeries.”

While the fellowship is not yet accredited (it is the very first of it's kind), Dr. Dronka has already benefited with career opportunities through her year of surgical training as well. This fall she'll be joining the University of Pikeville School of Optometry as their new Professor of Surgery. She's inspired to serve in training the next generation of optometrists on the techniques, procedures, and technology that will be essential to providing ocular healthcare for generations to come.

Women in Focus: Dr. Stephanie Aguilar on Being a Young Optometrist in 2018

As 2018 get's kicked off, it's been so much fun getting to meet more young female optometrists making waves in our profession through Eyedolatry. The stories of these women are so inspiring, especially since when I was a student optometrist I had no young female doctors I could look up to as a role model in navigating my new career. Times, they are changing, and none too soon! Review of Optometry published their 2017 salary review, and the gender pay gap for full time optometrists is an abysmal 40% (up 3% from 2016). In the face of a not-so closing gap in our profession's income, the number of female optometrists entering the workplace is at an all time high. These ladies will be leading us into a brighter future, not just in closing wage discrepancies and challenging the male-dominated representation in leadership across our industry and profession, but in how they innovate and expand the scope of our profession.

The optometric work force is changing! Women in Optometry reported last year that 66% of optometry school graduates in 2021 will be female. Dr. Stephanie Aguilar (above) is one of this new generation of optometrists that will be the first to see the profession switch from male dominated to female dominated.

Today we're featuring one such young female leader in optometry, Dr. Stephanie Aguilar of Alamo Eye Care in San Antonio, Texas. She's an associate doctor at a private practice that is also owned by a female optometrist, Dr. Melanie Frogozo. Female doctors working for female doctors is a relatively novel change within our industry, and it's been so exciting to see more female optometrists owning their own private practices, and hiring the next generation of young women as their associates!

After earning her doctorate from the InterAmerican University of Puerto Rico in 2015, Dr. Aguilar pursued her residency in ocular disease and primary eyecare at the VA Boston Healthcare System. Why did she decide to pursue optometry? She was exposed to the profession at a very young age. "Every year I went to the optometrist, I enjoyed going because my doctor was sweet and I got to pick out cool glasses that helped me see better. In college I shadowed a few of my dad’s MD friends, knowing I wanted to go into the healthcare field. I asked my optometrist at the time if I could shadow him. He agreed and I started an unpaid internship working part-time. I LOVED it. He became my mentor and asked me if I could see myself becoming an optometrist and being happy for the rest of my life."

The decision to pursue residency training after completing optometry school can be a difficult one. Dr. Aguilar knew by her third year in school that it was the right decision for her. "All of the coolest speakers (whether AOA representatives, private practice doctors, etc.) that came to school had done residencies and spoke so highly of the experience. My professors in school also recommended a residency to improve my clinical skills, as well as gain knowledge and experience before going into the workplace. At that time, I was also considering a career in a hospital setting and knew if I wanted to ever do that, a residency was a must."

She has great advice for student ODs that are interested in pursuing a residency of their own. "The best way to prepare for residency applications is to start early. The easiest ways to stand out are to be involved in your school and excel in the areas that your residency is in." During the interview, be prepared to discuss specifics about your clinical experiences so far, and be personable and conversational throughout the process. "Think of a few clinical scenarios that were significant to you, as this was a common topic of conversation during interviews. As always, be yourself because it’s likely that during your interview you are not only being evaluated on your credentials, but also whether or not your preceptors can stand working with you for a year!"

Finding the right job out of school or residency is a huge milestone (or hurdle!) in a young optometry career. While Dr. Aguilar had entered her residency considering working in a hospital setting, it was a private practice back in her home town of San Antonio, Texas that became the right fit for her. Like many great opportunities, networking was the key. "In my case, who I knew was critical. I found my job through a previous volunteer experience. Every time I came home from optometry school, I would volunteer at the Haven for Hope I Care Clinic that provides eye care for the homeless population in San Antonio. I had a good relationship with the manager at the office. While in Boston, I called her and asked if she knew any doctors hiring in the area. She put me in contact with two doctors, one of them being my place of work now. My advice is to treat everyone with respect because you never know who will help you down the road. Keep your options open, interview with many different offices and always ask for what you deserve in compensation." With our pay gap as it stands currently in optometry, her words of advice can't be more of a call to action!

Patient care is Dr. Aguilar's true passion, and it hasn't taken long in her young career to see her love for optometry touching lives. Even after a residency focused on treating and managing ocular disease, it's the simple day to day mission of being an optometrist - helping patients see their best - that has left the biggest impression on her so far. "The patient encounters that impact me the most are those in which I feel I’ve made an improvement in the patients’ lives. Whether it’s a patient that is successfully fit into multifocal contacts, a child seeing their parents clearly for the first time thanks to new glasses, or a corneal ulcer patient that is healed after two weeks of pain and blurred vision."

The future for young optometrists hold exciting promise, but plenty of challenges as well. "In my opinion the biggest immediate risk to our profession is telemedicine or optometry offices that offer a “comprehensive eye exam”, when in reality it’s someone remotely performing refraction and then reviewing anterior and posterior segment photos."

"Another risk to our profession is the group mentality that someone else will take care of this problem. In my opinion, all optometrists should support the American Optomteric Association because they work tirelessly to support and improve our profession. Without them, there is no telling where optometry would be and I hope I never to find out. So being a member of the AOA, joining their team or lobbying on optometry’s behalf is advantageous to all of us."

Prescribing Glasses for Migraines and Recurrent Headaches

Deep, boring pain behind your eyes. Throbbing, pulsating pain in your temples. Vision changes, zig zags, flashing lights, or tunnel vision. Nothing seems to help except lying down or just closing your eyes. For the 13% of Americans living with chronic headaches and recurrent migraines, these symptoms are all too familiar. In fact, those diagnosed with chronic migraines spend 15 or more days each month with a migraine headache, and each episode could last 72 hours or more at a time (a condition known as status migrainosus). Photophobia or light sensitivity is one of the most common vision complaints of someone living with migraines, so seeking help from an optometrist can be a great resource for finding relief. There are numerous eyewear tints, coatings, and lens designs that could be a major source of comfort, and even a low powered prescription could greatly reduce eyestrain or tired eyes that could be a trigger for a migraine event.

What Causes Migraines?

If you are suffering from chronic headaches, the first agenda item has to be ruling out any health condition. Migraines are a diagnosis of exclusion, meaning all other health conditions must be ruled out first. Serious findings like tumors, hemorrhage or bleeding, vascular malformations, and increased intracranial pressure are all possible causes of recurrent headaches and need medical investigation. A thorough eye exam is a great place to start; through imaging and dilation, your eye doctor can assess the internal eye for any signs of swelling or bleeding that could be a tell-tale indication that a systemic health condition is at play that requires further imaging and work up. Blood work could reveal nutritional deficiencies that can majorly contribute to chronic headaches, including common culprits like low Vitamin D (10% of Americans have low Vitamin D levels due to our indoor lifestyles), low magnesium, and low Vitamin B6 and B12.

Although migraines are still not completely understood, scientists theorize they result from changes in blood flow to different areas of the brain. Stress, fatigue, low blood sugar, or some other culprit upsetting the normal flow of oxygen and nutrients within the body causes the release of serotonin and constriction of the blood vessels supplying the brain. This triggers a cascade of reduced blood flow throughout the brain's different regions, including those responsible for vision and pain. If you've ever been told to drink caffeine or eat a candy bar to help with your migraine, the idea here is that if you can increase blood flow back to the brain with a kick of stimulation like sugar or caffeine, you may be able to shorten the life of the migraine.

How Do Glasses Help?

The best way to prevent a migraine is to stop the trigger, but of course stopping stress or fatigue is easier said than done. This is where prescription eyewear could make a big difference. If you experience ocular stress or ocular fatigue, prescription eyewear can greatly reduce strain on the eyes. Do you feel that your eyes are sore or tired at the end of the day? Do you find yourself rubbing or having to close your eyes frequently at work? Do you have trouble with light sensitivity or oncoming headlight glare? All these are common complaints of someone suffering from an uncorrected visual prescription, even if you feel like you see "clearly" on a regular basis.

Low amounts of prescription that you wouldn't visually need for every day wear can make a huge difference for someone working a heavy computer job or in a profession with high visual demand. It's not uncommon for graphic designers, dental hygenists, research scientists, healthcare workers, teachers, accountants, or lawyers to spend the majority of their day doing extremely detailed near work, which can cause significant stress to the visual system. But with the average American spending over 10.5 hours a day on a screen, most of us are now falling into the category of high visual demand needs these days!

What type of glasses could help with near work eye fatigue? Besides prescriptions for specific computer or reading tasks, lens technology your doctor may recommend include:

Prism: Do you suffer from double vision when you get tired? Do you feel a pulling sensation or like your eyes are wandering apart? If your eyes struggle to work together fluently, it can be a major source of eye strain. Your optometrist will check your eyes for an eye turn (tropia) or anomalous eye posture (phoria) during your exam to uncover these issues. If the eyes under or over focus when doing near work, a prism prescription helps relax how much effort your eye muscles have to put into compensating every day. Tinted Lenses: Yellow lenses block all blue light, not just high energy blue wavelength light so there are some pros and cons.
